NAR Nasopharyngeal Airway (NPA) with Lubricant

When a casualty loses consciousness the muscles in the jaw and tongue relax and the tongue can fall back and close off the airway. It is one of the most common airway problems in trauma and one of the simplest to fix if you have the right tool on you. The North American Rescue Nasopharyngeal Airway is that tool: a soft flexible tube that goes in through the nostril and rests behind the tongue to hold an open path for air.

Because it follows the natural path of the nasal passage instead of going over the tongue, an NPA can be tolerated by a patient who is semi-conscious or who is unconscious with the gag reflex still intact. That is the window where it does its work and it is why an NPA shows up on most IFAK packing lists.

NAR packages each one with a sealed packet of surgical lubricant attached, so everything you need for insertion comes out of the kit in one grab. You are not digging for a separate lube packet while the airway is still closed.

An airway only works if it fits the person it goes into, which is why we stock the full run from 24F to 36F in even sizes. The 28F is the size issued in most military IFAKs and the one to grab if you are building a single kit for an average adult. The 24F and 26F fit smaller adults and the 30F through 36F cover larger patients. If you are stocking an aid bag or a team kit, carrying two or three sizes lets you fit the casualty in front of you instead of making one size work. An NPA is a medical device and it belongs in trained hands. Know the contraindications, follow your protocols and if you carry one get hands-on time with it in a TCCC or TECC course.
